Cigna Global Health Options is modular. You choose a core inpatient plan at one of three levels, then add only the modules you want. Silver has a US$1 million annual limit for inpatient and day-patient treatment, Gold US$2 million, and Platinum pays in full. All three include cancer care, mental health support and telehealth.
The optional modules are International Outpatient (from US$15,000 a year on Silver up to paid in full on Platinum), Medical Evacuation and Repatriation, Health and Wellbeing, and Vision and Dental (annual maximums from US$1,250 to US$5,500). You can choose worldwide cover or worldwide excluding the USA, which makes a large difference to the price.
